Rationale for remodelling annuloplasty to address functional tricuspid regurgitation during left-sided valve surgery.

چکیده
The surgical management of functional tricuspid regurgitation is controversial. Functional tricuspid regurgitation is common in patients with aortic and mitral valve disease, particularly in the presence of pulmonary hypertension. The high incidence and reversibility of functional tricuspid regurgitation are due to the unique compliance of the thin right ventricular wall: modest increases in right ventricular preload, afterload, and dysfunction can result in significant functional tricuspid regurgitation. Surgical management is controversial primarily because of uncertainty over the long-term course and clinical sequelae of tricuspid regurgitation after leftsided valve surgery; and the diagnostic challenges presented by this very dynamic dysfunction.

Right sided heart evaluation after successful mitral valve replacement.
Introduction: It is well-documented that right-sided heart dysfunction and significant tricuspid valve regurgitation (TVR) have adverse effects on patient outcomes after left-sided heart valve surgery. Therefore, the evaluation of right ventriclular (RV) function and TR severity in patients who had undergone mitral valve replacement (MVR), associated with/without concomitant su...

7. Clinical bottom line TV insufficiency should be treated during left-sided valve surgery when TR annulus is dilated (G21 mmym ;)70 mm 2 intra-operatively; G3.5 cm at trans-thoracic echocardio-graphy (TTE) w18x) regardless of the absolute grade of regurgitation, in cases of preoperative AF, trans-tricuspid PMK lead and underlying rheumatic disease. Also, 'ring' annuloplasty techniques should b...
متن کاملSurgical management of functional tricuspid regurgitation with a new remodeling annuloplasty ring.
BACKGROUND Moderate-to-severe functional tricuspid regurgitation (TR) should be corrected in patients undergoing surgery for left-sided valvular diseases, to improve long-term outcomes. Several techniques of surgical repair (suture annuloplasty or prosthetic annuloplasty) to correct this condition have been described. Multiple clinical studies have shown the superiority of prosthetic remodeling...
متن کاملeComment. Adjustable tricuspid annuloplasty for functional tricuspid regurgitation.
Functional tricuspid regurgitation (FTR) is a neglected and underestimated pathology. It occurs mostly from annular dilatation and right ventricular enlargement, which is frequent secondary to left-sided heart failure from myocardial or valvular causes, right ventricular volume and pressure overload, and dilatation of the cardiac chambers. Moderate-to-severe FTR should be corrected to improve s...
متن کاملManagement of tricuspid regurgitation
Secondary tricuspid regurgitation is the most frequent type of tricuspid insufficiency in western countries. Its surgical treatment is still an object of debate both in terms of timing and surgical techniques. Until recently, the avoidance of surgery for tricuspid repair was commonly accepted in patients with less than severe secondary tricuspid regurgitation undergoing left-sided valve surgery...
